DIVING ACCIDENT AT DESCEND DTE

ADAS regrets to announce that an accident occurred on 15 September 2006 at Descend UTC in which a trainee diver died.  It appears that the diver ran out of air at the same time as his lifeline became entangled and the diver did not activate his reserve air supply.  ADAS would like to pass on our deepest sympathy to the family.

The accident is being investigated by the NSW police as agents of the Coroner and by the NSW occupational health and safety regulator (the WorkCover Authority).  A preliminary investigation has been undertaken and the dive site has been inspected and cleared by the WorkCover Authority Diving Inspector and the NSW Police Diving Unit.  The Workcover Authority has authorised the resumption of the dive training.

A full investigation of the circumstances of the accident is being conducted by the relevant authorities and ADAS will consider and act on the findings of the investigation when it is concluded.  In the meantime, ADAS, in conjunction with the Diver Training Establishments, is reviewing all relevant policies and procedures and training and assessment arrangements to ensure that they are at the standard of best practice.
